Intake Form
To begin the scheduling process, provide us with your first and last name, phone number, UW email address, and a short message. Filling out the contact form does not automatically qualify you for an appointment. As we only serve currently enrolled students that have paid the Services and Activities Fee (SAF), this form is only accessible to current UW students logged in with their UW NetIDs.
We do not provide legal advice by email or phone, and walk-ins and same-day appointments are not accepted. After submitting your information in the contact form, SLS staff will review your message and send you a personalized link to the intake form where you can provide more details about your situation. SLS cannot provide same-day consultations, nor can we provide legal advice by phone or email. If you are unable to access or complete the intake, please call our office at (206) 543-6486 or email us at slsuw@uw.edu.
After the intake form has been completed, SLS staff will screen your intake to confirm your eligibility and identify any conflicts of interest. If no conflicts arise, a separate link will be emailed to you to schedule your appointment. After your appointment has been scheduled, you will be emailed a link to our Conditions of Consultation form, which must be filled out electronically prior to your appointment.
